Here, you will find out how to apply the NTTB tooth samples for your research and the various templates you will need for your study approval. Please follow the workflow below.
NTTB Application Workflow
Step 1
Use Checklist #1 to determine which Institutional Review Board (IRB) to apply to.
Step 2
Click below for more information on how to apply to the IRB.
State in ‘Section F’ and 'Section S' that the study team will be receiving extracted teeth or stem cells from NTTB.
Attach the NTTB approval email, extracted teeth or stem cells informed consent form (ICF), and DSRB registered letter to the application.

Step 3
After receiving your study approval, click here to fill up the NTTB service form. Please contact us at 9004 1044 for the password.
Step 4
You will be informed through email when the teeth or stem cell samples are ready for collection.
