Research Laboratory Application

All staff and students intending to use the research laboratories must apply for approval before using our laboratories, offices, and facilities. This ensures that users comply with all relevant NUS’ and NUS Dentistry’s safety policies.

For supervisors and applicants, please follow the workflow below. Laboratory access will not be given until “Approval to Use Research Lab” has been issued. During the transitional period, applicants can still go to the research laboratories and observe research works, but they will need to be supervised by authorised users as instructed by their supervisors.

Step 1

For supervisors

The supervisor will initiate the application procedure for laboratory use.

1. Complete the Qualtrics e-form to send the applicant the required information to apply for laboratory use. Please contact Dr Liu Fengxia (denlf@nus.edu.sg) for the password.

Once the e-form is submitted, the new applicant will receive an auto-generated email with the required information.

Step 2

For applicants

The applicant must submit the laboratory use application after receiving an auto-generated email signed off by their supervisor.

1. Complete the following items listed in the email sent to all applicants:

  1. Safety online modules (in Canvas, CHRS, & NUH Elearn Portal);
  2. General research lab orientation;
  3. Authorisation form – Occupational Health Services in NUS.

2. Once you have completed the items above, fill up the laboratory use application e-form (the link will be provided in the supervisor’s email) and upload all required documents.

Once the application is submitted, you will receive a Qualtrics-generated acknowledgement email.

Thereafter, you will receive an email regarding your application's outcome from the Safety Unit within three (3) working days.
